  • The addition of a warning statement that any misrepresentation in the PDS and the work experience sheet shall cause the filing of administrative/criminal case/s against the person concerned.



  • The removal of some fields that are deemed unnecessary or sensitive, such as height, weight, blood type, religion, spouse's occupation, father's/mother's occupation, etc.



  • The inclusion of some fields that are deemed relevant or important, such as email address, mobile number, government-issued ID number, etc.



  • The modification of some fields to make them more specific or clear, such as date of birth, place of birth, citizenship, etc.



  • The rearrangement of some sections and fields to make them more logical or consistent, such as moving the civil service eligibility section before the work experience section, etc.

How to Fill Out CS Form No. 212 Revised 2017?

  • Write your date of birth in mm/dd/yyyy format. For example, 01/01/1990.



  • Write your place of birth in the following order: city/municipality, province/state, country (if abroad).



  • Write your sex as either Male or Female.



  • Write your civil status as either Single, Married, Widowed, Separated, or Annulled.



  • Write your citizenship as either Filipino or Dual Citizenship. If you have dual citizenship, write the details of your other citizenship in the space provided.



  • Write your height in meters. For example, 1.65 m.



  • Write your weight in kilograms. For example, 60 kg.



  • Write your blood type as either A, B, AB, or O. Include the Rh factor if known. For example, A+ or O-.



  • Write your GSIS ID number, PAG-IBIG ID number, PHILHEALTH number, and SSS number if you have them. If not, write "N/A".



  • Write your residential address in the following order: house/block/lot no., street, subdivision/village/barangay, city/municipality, province/state (if abroad), ZIP code.



  • Write your permanent address in the same order as above. If it is the same as your residential address, write "Same as above".



  • Write your telephone number, mobile number, and email address if you have them. If not, write "N/A".
